This page explains what we collect through landmarkchiropractic.com, how we use it, and the choices you have. We've kept it plain and short on purpose.

At Landmark Chiropractic, we respect your privacy and only collect the information we need to answer your questions, schedule your visits, and care for you well. This Privacy Policy applies to this website. It does not, by itself, govern the protected health information we handle as your healthcare provider — see Section 3 for how those rules differ.

This policy is published by Landmark Chiropractic, LLC ("Landmark Chiropractic," "we," "us," or "our").

01 Information we collect

We collect information in two ways: what you give us directly, and what's gathered automatically when you use the site.

  • Information you provide. When you fill out a contact form, request an appointment, or submit intake paperwork, you may give us your name, email address, phone number, and the details of your message or request.
  • Health information you submit. Our new-patient intake forms ask about your health history and symptoms. This is sensitive information, and we treat it that way — see Section 3.
  • Information collected automatically. Like most websites, our server and tools may log your IP address, browser and device type, the pages you visit, and the date and time of your visit. This helps us keep the site working and understand what's useful.

02 How we use your information

We use the information we collect to:

  • Respond to your questions and requests;
  • Schedule, confirm, and manage your appointments;
  • Provide and document your chiropractic care;
  • Send you information you've asked for or that relates to your care;
  • Operate, secure, maintain, and improve the website;
  • Comply with our legal and professional obligations.

We do not sell your personal information, and we do not use it for purposes unrelated to those above without telling you first.

03 Health information & HIPAA

As a healthcare provider, we follow the Health Insurance Portability and Accountability Act (HIPAA) and Texas law when we handle your protected health information (PHI) as part of your care.

Important — please read

This website Privacy Policy is not the same as our Notice of Privacy Practices. The Notice of Privacy Practices is the separate document — provided to you at your first visit — that describes your rights and how we use and protect your protected health information in the clinical setting under HIPAA.

Where this Privacy Policy and our Notice of Privacy Practices both apply to the same information, the Notice of Privacy Practices controls how we handle your protected health information.

When you submit health details through our online intake forms, that information travels through our forms provider (see Section 4) before it reaches us, and is then maintained as part of your patient record.

04 Third-party services we use

We use a small number of trusted service providers to run the site and the practice. Each handles your information only to provide its service to us, and each has its own privacy policy:

  • SKED — our patient scheduling and appointment platform (portal.sked.life).
  • Jotform — our online intake-paperwork forms, including new-patient health history.
  • Google — Google Maps (the map on our Contact and location pages) and Google Fonts.

We encourage you to review those providers' privacy policies to understand how they handle information on our behalf.

05 Cookies & analytics

Cookies are small files a website stores on your device. We use cookies and similar technologies to keep the site working properly and to understand how visitors use it so we can improve it.

We use analytics tools — such as Google Analytics — that collect anonymized, aggregated information about site usage. You can control or delete cookies through your browser settings; turning some cookies off may affect how parts of the site work.

06 How we share information

We do not sell your personal information. We share it only in these limited situations:

  • Service providers who help us run the site and the practice (Section 4), under agreements that limit how they may use it;
  • For your care, such as with another provider you ask us to coordinate with;
  • When the law requires it, such as a valid subpoena, court order, or public-health obligation;
  • To protect rights and safety, including enforcing our terms or preventing harm.

07 Data security

We use reasonable administrative, technical, and physical safeguards to protect the information we hold. No website or method of transmission is completely secure, however, so we cannot guarantee absolute security. Please don't send sensitive health details through an unsecured channel such as plain email — use our intake forms or call the office instead.

08 Data retention

We keep your information for as long as needed to provide your care, run the practice, and meet our legal, professional, and recordkeeping obligations. Patient health records are retained for the period required by Texas law and applicable healthcare regulations. When information is no longer needed, we dispose of it securely.

09 Your choices & rights

You can:

  • Ask us what personal information we hold about you and request a copy;
  • Ask us to correct information that's inaccurate;
  • Opt out of non-essential marketing messages at any time;
  • Request access to or amendment of your health records, as provided under HIPAA and Texas law.

To make a request, contact us using the details below. We may need to verify your identity before we act on a request.

10 Children's privacy

This website is intended for adults and is not directed at children under 13, and we do not knowingly collect personal information from children through the site. When we care for younger patients, intake and scheduling information is provided by a parent or legal guardian. If you believe a child has given us information through the website, please contact us and we'll remove it.

12 Changes to this policy

We may update this Privacy Policy from time to time. When we do, we'll change the "Last updated" date at the top of this page. If we make a significant change, we'll take reasonable steps to let you know. Your continued use of the site after an update means you accept the revised policy.
